According to a press release from Qualitest Pharmaceuticals and the FDA, two lots of Accusure Insulin Syringes are being recalled due to defective needles. The needles can detach from the syringe during use, causing injury by becoming stuck in the insulin vial, pushing back into the syringe or remaining in the skin after an injection.

The syringes are 31G - short needle - either 1/2 cc or 1cc, with lot numbes 6JCB1 or 7CPT1. If you have these syringes, do not use them. Contact Qualitest at 1-800-444-4011 for product replacement instructions.

The lot number appears on the white paper backing of each individual syringe. This is a nationwide recall.
